RN Case Manager
The ideal Candidate will be a
Registered Nurse with a minimum of one year’s Hospice or Home Health
experience.
Candidate will be responsible for managing a caseload of patients
that includes addressing patient care needs through telephone
consultation and home visits. Applicants must possess a valid RN
license for North or South Carolina and a current driver’s license.
The ability to work independently is imperative.
Helping Hands Hospice is non-profit accredited hospice agency
serving Chesterfield, Darlington, Dillon, and Marlboro Counties.

Weekend on Call Nurse
The ideal Candidate will be a
Registered Nurse with a minimum of one year’s Hospice or Home Health
experience.
Candidate will be responsible for maintaining scheduled visits over
the weekend, as well as taking call for the weekends. This is a full
time position. Applicants must possess a valid RN license for North
or South Carolina and a current driver’s license. The ability to
work independently is imperative

Medical Social Worker
The ideal Candidate will be a Licensed
Social Worker with a Bachelor’s Degree in Social Work from an
accredited School of Social Work, with a minimum of one year’s
Hospice or Home Health experience preferred.
Candidate will be responsible for
managing a case load of patients that includes social service
activities as it relates to terminally ill patients and their
families, including treatment, planning, and counseling with other
staff and volunteer members of the team.

Volunteer Chaplain
Provide support services to terminally
ill individuals and their patients. This volunteer position will
coordinate a program of spiritual support for patients and their
families, and assist with bereavement services.
